Students 7-8 grade need an activity physical/consent form that is signed by physician AND parent. Forms need to be turned into the Office before the first practice.
Students 9-12 grade need an activity physical/consent form that is signed by a physician AND parent in order to participate in any NSAA Activity, including fine arts. Forms need to be turned into the Office before the first athletic or activity practice.
